Insurance and costs, in plain words
We take Medicare and most health plans. Before your first visit, we call your plan and tell you what you will pay.
Coverage we accept
Medicare
Original Medicare Part B covers outpatient physical therapy when your therapist documents that you need it.
Medicare Advantage
We work with most Medicare Advantage plans in West Michigan. Some ask for a referral first.
Employer health plans
Most major commercial plans, including PPO and many HMO plans.
Medicaid health plans
We accept several Michigan Medicaid health plans. Call to check yours.
Workers' compensation
Hurt at work? Bring your claim number and your employer contact.
Auto accident claims
We bill Michigan no-fault auto insurance with your claim number.
We check your benefits for you
You share your card
Read the numbers to us on the phone, or bring the card to your first visit.
We call your plan
Within one business day, before your first visit.
We call you back
You hear what you will pay per visit, in dollars.
A benefits check is our best estimate. It is not a promise of payment. Your plan makes the final decision.

Insurance words, explained
Four terms you will hear when we call you about costs.
- Copay
- A set amount you pay at each visit, such as $25. Your plan pays the rest.
- Coinsurance
- A share of the cost you pay, such as 20%. Original Medicare usually works this way after your yearly deductible.
- Deductible
- The amount you pay each year before your plan starts to pay its share.
- Referral
- A note from your doctor. Most patients do not need one to start, but some plans ask for it.
